In our office, we offer high quality care and payment options that work for all of our patients. Due to the specialized nature of our pratice, our fees may be higher than expected. Although we are NOT in network with any insurance company (or plan), we will be happy to file your insurance as a courtesy. We usually estimate the expected benefit and ask you to pay the difference at the time of treatment. Below you will find a list of insurance companies we work with.

Please note: Payment is due at the time of service.

Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
